If I find out that I have been in close contact with someone who has COVID-19 or that I test positive for COVID-19, I promise to  notify the Director of the Plymouth Community Food Pantry immediately, by phone (860)-584-1750 or email foodpantry@plymouthfoodpantry.org.  I will provide my name, the date I tested positive or came in contact, the date of my most recent visit to the Pantry, and a brief description of the general circumstances surrounding the contact or contraction of the virus.  Please do not share the names of any person other than yourself.
